How to claim

How to claim the personal accident and/or fire assistance benefits?

Claimants must present all required and valid documents listed under each type of claim at any Cebuana Lhuillier Pawnshop branch.

NOTE: Only claims below P50,000 are payable in cash. Claims above P50,000 shall be thru check payment.

List of requirements:

• Requirements for FIRE INSURANCE CLAIMS

1. Original Certificate of Insurance or its blue form duplicate copy
2. Picture(s) of burned property
3. Barangay Certification of Fire Loss / Incident indicating details of insured’s (OC)
4. Bureau of fire Prevention Certification indicating details of fire incident (OC)
5. Claimant’s valid ID’s w/ Photo, Signature, and Address (photocopy)

• Requirements for ACCIDENTAL DEATH & DISMEMBERMENT CLAIMS

1. Original Certificate of Insurance or its blue form duplicate copy
2. Death Certificate of Insured (OC, CTC, NC)
3. Police Report &/or Incident Report (OC)
4. Claimant's Valid ID’s w/ Photo, Signature and Address (photocopy)
5. If parent is claimant, Birth Certificate of insured (photocopy).If son or daughter is claimant, Birth Certificate of claimant (photocopy). If sibling is claimant, Birth Certificate of both insured and claimant (photocopy)

 

• Other requirements that may be requested from the claimant, if deemed necessary by CLIS personnel.

1. Medical Certificate (OC, CTC, NC)
2. Medical Discharge (OC, CTC, NC)
3. Autopsy Report (OC, CTC, NC)
4. Other documents that insurer may require to support claim
